What this is


Most risk registers are a list of things that could go wrong. This one puts a number on each of them, twice: the risk as it stands when nothing is written down, and the same risk when the record exists. The gap between those two numbers is the entire argument for documentation, and it is the only thing this library is trying to show you.

Scored, not asserted

Likelihood × Impact on a 5×5 matrix, modeled per NIST SP 800-30. Every band shift says which band moved and why.

Sourced, or labeled

Documented, industry benchmark, or estimate. Estimates say so and carry their assumptions. Nothing is invented to fill a card.

Vendor-neutral

Software appears only inside clearly fenced implementation blocks. The risks don’t care what you run.

Published counts are stated honestly. A trade goes live once its Critical and High scenarios carry verified evidence — the remaining scenarios follow in waves rather than being padded out to look complete.

How to use it


Filter to your role and read the four or five scenarios that touch your seat. Every scenario has a permanent anchor you can link to, send to a foreman, or cite in a meeting — for example, R-SAF-007 is the confined space rescue plan gap on the mechanical page. Come back to it before the next entry, the next pick, or the next shutdown window.
